Application forms

This is where you can find our most requested application forms.
Unless otherwise specified, you will need to print and fill in the form and then take it to your nearest Westpac branch in order to complete the application. Remember to bring suitable identification with you too.
	If you need some help deciding which account or credit card product is right for you, check out the information in Accounts and Services first or pop into your nearest Westpac branch.
	If you need Business Banking information and/or forms, visit Business Banking or contact a Business Manager.
Bank accounts
	Open a personal bank account form or you may be able to open one now through Online Banking if you’re a Westpac customer already.* Remember to bring suitable identification with you when bringing this form into branch and to specify which transaction and/or savings account you want.
	Open a Westpac Junior Account You will also need an IRD number application form IR 595 if your child doesn’t have an IRD number yet and proof of ID for your child, such as their passport or New Zealand birth certificate.
Existing Westpac customers only:
	Make changes to your account (such as adding or removing a signatory).
	Authorise the set up of an automatic payment.
Credit cards and DebitPlus Visa
	Fill in a Westpac credit card account application form or apply online.
	Fill in a Low Interest credit card application form or apply online.
	Fill in a Gold Credit Card application form or apply online.
	Fill in a BusinessPLUS application form or apply online.
	Fill in a Titanium credit card application form or apply online.
	Fill in a DebitPlus Visa card application form or you may be able to apply online through Online Banking if you’re a Westpac customer already.*
Remember to bring proof of your identity, address and income when bringing your application form into branch and to specify which credit card account you want.
Existing Westpac credit card holders only:
	Request a change to your credit card account, such as increasing your credit limit, adding or removing a signatory or updating your name or address.
	Change your credit card type. Use this form to change between Westpac credit cards.
	Set up a credit card or CashLine direct payment.
	Apply to transfer your balance from another bank’s credit card to a Westpac credit card with a low interest rate.
* Current Westpac customers
Did you know that once you’ve logged in, you may be able to do all of the following from within Online Banking:
	open a new everyday transaction or savings account
	apply for a DebitPlus Visa card and
	set up a new Term Deposit
Login to Online Banking now.
Note: some conditions apply and in some cases, further documentation is required.
Suitable proof of identification
You can bring along any one of the following current forms of ID:
	New Zealand Drivers Licence
	New Zealand Firearms Licence
	Passport
Or any two of these current forms of ID:
	NZ Birth Certificate
	NZ Certificate of Identity issued by NZ Immigration Services or Department of Internal Affairs NZ
	Community Services Card / SuperGold Card
	Marriage Certificate
	New Zealand Education Institute issued identification card containing photographic image of the card holder.
	Credit or debit card from another bank, bearing your name and signature.
